The road to long-term recovery is complex, and staying on track with it goes beyond the skills and strengths of most people, much less someone struggling with the beginnings of sobriety. Even when your loved one has wholeheartedly agreed to enter treatment after a successful intervention, the path is full of organizational challenges and potential frustration.
Those entering recovery must coordinate & attend various appointments, collaborate with various counselors & medical professionals on the care team, and, most important of all, maintain a schedule that lays down a foundation for health and self-improvement. There may also be various things to coordinate with other outside parties like probation officers, attorneys, employers and educators.
We offer oversight & support during your loved one’s recovery
Our intensive case management services offer a level of oversight during care and after-care that families typically cannot provide themselves for those in recovery. Our case managers are patient advocates that serve as the primary liaison, ensuring appointments are attended, any tasks are finished, and that goals and milestones are reached, all while keeping the family member informed.
We operate on a full-service basis, believing that it is best during this period if the patient and family work with a liaison and buffer. The coordination and management of after-care is stressful and can cause additional friction in the family dynamic. The buffer that intensive case management provides fully allows the patient and the family to each separately focus on their own healing and recovery process.
Coordination of the details of care, referrals & introductions to professionals, new services added or eliminated, and travel plans are among the Case Manager’s responsibilities. Other details that we cover include:

- Treatment Planning
- Coordinate care among additional treatment providers
- Drug and alcohol monitoring
- Accountability
- Introduction to peer support groups
- Liaison between family, client, and any outside treatment professionals and medical providers

An intervention for alcohol or drug addiction is a process and not just a one-time event. We work with each family individually to create a personalized plan that will assist them best on their loved one’s road to recovery.
Understanding
Our first steps in the intervention process are to understand our clients’ situation & recovery needs, so we can identify how to best provide support. A great deal of strategic planning & coaching occurs with the family prior to the actual intervention.
The Family Meeting
On the day of the intervention, we will meet at a pre-determined and neutral location. The client will be supported by participating family members & close friends.
Treatment Center
After the intervention meeting, we will notify the treatment center that your loved one has chosen to attend rehabilitation and will be starting immediately.
Sober Transport
During this part of the process, our last step is to make sure the client arrives at the treatment center safely, unless other arrangements have been determined prior.
You don’t need to wait for your loved one to hit rock bottom. The role of a professional interventionist is to raise the bottom.
